Why Botox Dose and Duration Differ by Area: Smoothing Wrinkles vs. Slimming Muscles

At a glance
Botox uses the same drug for two different goals: relaxing small facial expression muscles to soften wrinkles, or reducing the volume of thick muscles to refine the contour. The dose and the way changes appear differ by area, so knowing this in advance makes the decision easier.
Reducing muscle volume: square jaw and calves
Botox is also used when a well-developed masseter (the chewing muscle) makes the jawline look square, or when the calf muscles stand out. The goal here is to reduce the volume of the muscle itself. This needs a much larger dose than wrinkle treatment, and the change in shape appears gradually.
- Square jaw: The jaw contour starts to soften from around one month.
- Calves: The leg line changes over several weeks as the muscle shrinks.
Smoothing wrinkles: forehead and between the eyebrows
Horizontal forehead lines and vertical frown lines between the eyebrows are dynamic wrinkles that form when the face moves. Weakening the movement of the muscles that create them makes the wrinkles fade. This needs a smaller dose than muscle reduction, and the effect usually becomes clear within 2 weeks.
Too much can make your expressions look unnatural or give a drooping-eyebrow feeling, so if you want a natural look, the dose needs to stay appropriate.
How long the effect lasts: 3–6 months
The effect is not permanent. It usually lasts 3–6 months, after which the muscles regain movement and things gradually return to how they were before treatment. As repeated treatments weaken the muscle, the interval until the next treatment can become longer.
Start with a small dose
People respond differently even to the same dose. That is why the safe approach is to do the first treatment at a conservative dose, check the result after 2 weeks, and then top up where needed. If a large amount is injected from the start and your expressions stiffen or the two sides look different, it is hard to undo.
What to do on the day of treatment
The first few hours after the injection are when the drug settles in place. Rubbing the area or lying down during this time can make the drug spread to places it was not intended to go.
- Do not lie down for about 4 hours, and do not rub the treated area.
- On the day, avoid saunas, strenuous exercise and alcohol.

Frequently asked questions
Will my expressions look frozen after treatment?
With an appropriate dose, natural expressions remain. A stiff feeling mainly happens when too much is used, and it goes away as the effect wears off.
How much does it hurt?
It feels like several short pricks with a fine needle, so it does not hurt much. If you are sensitive to pain, numbing cream is sometimes applied before treatment.
How often should I get it again?
Generally, treatments are repeated every 3–6 months, as the effect wears off. Rather than getting it too often, it is better to time the next treatment for when the effect fades.

When will I see changes from square-jaw or calf Botox?
Because these treatments reduce muscle volume, changes appear gradually. For a square jaw, the contour starts to soften from around one month, and for calves, the line changes over several weeks.
